I gotta be honest, as far as "plot twists" go, its certainly not gonna be in the top 50, or probably even of the greatest plot twists of all time if we are going to include movie storylines and video game storylines together.
For a twist to work well, its gotta make good "rational" sense when its put infront of the viewer. The storyline with Liquid trying to secreatly use Solid as a pawn, in order to get someone to be able to activate Metal Gear Ray is certainly not very rational in sense making. What if Liquid didnt survive the chopper exsplotion and then fall, from Solids stinger missiles?. The twist with Decoy Octopus pretending to be the Darpa chief was good though. I played the game more last night and watch the torture scene where Snake is captured and Volgin is torturing him. When Volgin commands The Boss to prove her loyalty, you can see she is reluctent to do anything lethal to him, and you can see she is reluctent to render him blind in both eyes.
I also think the reason she threw Snake off the suspension bridge is because she knew Snake would survive it, but at the same time convince Volgin that it was a genuine assasination attempt.
